This agreement, between                                  (the client) and Self Help Coaching, will begin on                    and continue for three months. After that, the client and coach will reevaluate the needs of the client on a monthly basis.

The fee for the Intake Session is NIS 500.00 with the fee for the initial three months being NIS 800.00 per month based on four (4) appointments per month. Payment by cheque or credit card (Visa, Isracard or Mastercard) due in advance on the first of the month and made payable to Judith Schwarcz. Subsequent to payment, a receipt will be forwarded to the client by post. The client is responsible for payment for all telephone calls. Calls can also be made via Skype to Judith2005 (calls are free via Skype).

Sessions can be held either in person, or over the telephone. Rescheduling of sessions, either by the coach or client, must be made at least 24 hours in advance, either by phone or email. Payment will be incurred by the client for any sessions missed and not cancelled at least 24 hours in advance.

Coaching sessions are jointly designed with the client. Coaching is NOT instead of therapy or counselling. It is a partnership involved in creating a powerful relationship that fits the client’s working and learning styles. Clients learn that they are in control of the relationship and of the changes they make in their lives.

Strict confidentiality of all sessions is guaranteed by the coach. Self Help Coaching reserves the right to seek second opinions from other certified coaches or professionals if deemed necessary. The client's name will not be mentioned in these discussions.

Our signatures on this agreement indicate full understanding and agreement of both parties to everything herein.

Once the agreement has been signed, the first session will be the Intake Session.
